Transitioning from youth to adult services is a process that requires planning. When a student turns 18, children services will end and new adult services may begin. A plan towards successful transition should begin as early as possible and educators can help prepare students for adulthood. Transition planning process is a chance for everyone (student, family, teachers, and administrators) to develop a vision with the students and their family to ensure their long-term success.
